• 1.   Présentation 

Le Centre d’Investigation Clinique Interrégional Antilles Guyane (Inserm CIC1424) a été créé en janvier 2008 avec l’intitulé «maladies infectieuses, génétiques et émergentes en zone tropicale». Il couvre les trois départements français d’Amérique : Guyane, Martinique et Guadeloupe. Un CIC mais trois sites: Guyane (Centre Hospitalier de Cayenne), Martinique (CHU de Fort-de-France), Guadeloupe (CHU de Pointe-à-Pitre).En Guyane, le CIC est situé dans l’hôpital de Cayenne, favorisant les échanges avec les différents services de soins.

L’existence du CIC Antilles Guyane se justifie par le besoin de mieux connaître les caractéristiques épidémiologiques des problèmes de santé propres aux Départements Français d’Amérique liés :

- à la diversité génétique des populations : origines multiples (amérindiennes, africaines, asiatiques, européennes)

- à la complexité / intensité des migrations régionales

- à l’émergence des « maladies de civilisation » et du « développement »

- à l’environnement tropical et la persistance d’agents pathogènes spécifiques.

Les axes thématiques forts du CIC1424 Antilles-Guyane concernent principalement les maladies infectieuses (paludisme, dengue, Zika, Chikungunya, infection par le VIH, histoplasmose), les sciences humaines (inégalités de santé, comportement et santé), et les maladies chroniques et génétiques (insuffisance rénale, maladie cardio et cérébro-vasculaires, maladie de Pompe, drépanocytose). La vision du CIC est d’être à la fois localement utile tout en visant l’excellence internationale.

 

• 2.   Compétences spécifiques et savoir-faire

Le CIC Antilles-Guyane a pour mission de prendre en charge les aspects méthodologiques des projets de recherche clinique mais aussi de les concevoir et de les mettre en œuvre. Il participe également à la mise en place et à l’évaluation de stratégies de santé publique.

En Guyane, le CIC1424 a pour mission de dynamiser la recherche biomédicale et épidémiologique ainsi que participer activement à l’organisation de la recherche médicale hospitalière. Parmi ces compétences spécifiques on retrouve :

- Méthodologie des études : conception des protocoles; conception bases de données cliniques (Capture System®), plan d’analyse et exploitation données, rédaction des rapports statistiques

- Aspects réglementaires : préparation protocole et soumission aux autorités, compétentes: CCTIRS, CNIL, ANSM, CPP, Comité d’éthique (CEEI), gestion échantillons- collections biologiques

- Gestion et coordination des études : mise en place dans les sites investigateurs; suivi des recherches, coordination des intervenants (investigateurs, laboratoires, CRB, autorités de santé)

- Assurance qualité : système qualité et procédures opératoires standards en conformité avec les BPC, assurance qualité des données; monitoring sites investigateurs; suivi des EIG; formation des intervenants sur site à la recherche clinique.
